Water, It's not just a drip. F327 is slowing rising to the top ten tags cited. Adequate hydration becomes a challenge in vulnerable populations. Health providers must have systems in place to ensure that their residents, are hydrated based on their assessed fluid requirements. Residents with co-morbidities such as not being able to communicate their needs, having swallowing difficulties/ on thickened fluids, or are dependent on staff for their fluids, are at a greater risk of being dehydrated. MDS triggers alone do not meet the facility's responsibility for screening residents at risk of dehydration. Follow the RAP Guidelines to rule out all causative factors and then care plan.
Standards of practice must be peer reviewed and evidence-based. Examples of hydration standards of practice are: The American Medical Director's Association (AMDA), Dehydration/Fluid Maintenance, The American Dietetic(ADA) Association's The World of Hydration; or The University of Iowa's Practice Guideline on Hydration Management. When determining policies and procedures, per F501, include your medical director and your director of nursing. Although there are several ways to calculate fluid needs, these standards use the 30 ml/kg calculation.
Boyer and Associates will be presenting a Webinar on Hydration, which will include how to prove that dehydration is unavoidable and what the state and federal surveyors look at, when determining non-compliance.
